Level One: $300

Level Two: $500
May include daily assist with grooming, daily assist with dressing, medication provision in apartment 3-5x/day, assistance with shower 3x/week, assistance to bathroom 1-3x/day, physician contact more than 3x/month, one-person assist with transfer, supervision and cueing with meals, plus other supervision, intervention and assistance.

Level Three: $700
Two person daily assist with dressing and grooming, daily assistance with treatment protocols, medication provision in apartment more than 5x/day and PRN, continuous supervision when dining, plus other supervision, intervention and assistance.

Rates and fees do not apply to Medicaid Waiver recipients.
